Neck pain after a road accident

Neck pain and stiffness that begin in the hours or days after a car or two-wheeler accident. The neck was moved quickly further than it is used to, and the muscles and joints react. It is genuinely painful and it almost always settles.

Do not wait for an appointment

Uncommon — but these need to be seen the same day.

  • Weakness, numbness or pins and needles in either arm
  • Difficulty walking, clumsiness in the hands, or a change in bladder control
  • Severe pain in the middle of the neck that is tender to press on the bone itself
  • A blow to the head, a period of unconsciousness, vomiting, or confusion that follows
  • Pain that is getting worse day by day rather than easing after the first week

Do this

  • Keep the neck moving from the first day, gently and within comfort. This is the single most important thing on this page
  • Expect it to be worse on the second and third days than on the first. That is the usual pattern and not a sign of deterioration
  • Use a hot pack and simple painkillers so you can keep moving, rather than to rest more comfortably
  • Go back to your normal routine as soon as you reasonably can, including driving when you can turn your head to check
  • Sleep with one supportive pillow, not two or three stacked

Avoid this

  • A soft collar. It feels protective and it prolongs the problem — neck muscles weaken quickly when held still, and people who wear a collar take longer to recover than people who keep moving
  • Resting in bed for days
  • Forceful manipulation or cracking of the neck in the early weeks
  • Strong massage while it is acutely painful
  • Reading the worst you can find online about long-term whiplash. The great majority of people recover fully, and expecting to recover is itself associated with recovering

The questions everybody asks

Why did it hurt more the next day?

That is how this injury behaves.

Pain often begins several hours after the accident and peaks over the next two or three days. People frequently walk away feeling fine and wake up stiff. It is not a sign that something was missed.

Should I wear a collar?

No, and this is worth being firm about.

Collars were routine for years and the evidence went the other way: people who keep the neck moving recover faster than people who immobilise it. If you have been given one, use it for a few hours at a time at most, not continuously.

Do I need an X-ray or a scan?

Usually not.

Where there is no midline bone tenderness, no nerve symptoms, and the neck moves, imaging does not change what we do. We X-ray when the examination suggests we should — not because an accident happened.

How long will it take?

Most people in six to twelve weeks.

Movement returns before the aching stops completely. A smaller number have pain beyond three months, and they do better with graded activity and a return to normal life than with further rest and investigation.

On cost: if we suggest a scan, an injection or an operation, ask us what it costs and what happens if you do nothing. We will answer both. You should never feel pushed into spending money.

Consultation is ₹500 for a first visit and ₹300 for a follow-up, the same at both clinics. For anything else, ask at the desk or on WhatsApp before it is done — we will tell you the exact cost.

Should you come and see us?

A page is a starting point, not a diagnosis. If the pain is not settling, come in and have it looked at properly — bring any X-ray, MRI or CT films you already have.
