Shin pain with running

Aching along the inner edge of the shin that comes on with running and eases with rest. It is the bone's covering and the muscle attachment reacting to a sudden increase in load, and it settles when the load is brought back down.

Do not wait for an appointment

These suggest a stress fracture rather than shin splints.

  • The pain becomes sharp and stays in one small spot you can cover with a fingertip
  • It hurts at rest, at night, or when you hop on that leg
  • The calf becomes swollen, hot or tight
  • The foot or toes become numb or weak
  • The pain came on suddenly during activity rather than building over weeks

Do this

  • Cut the running back rather than stopping altogether. Reduce distance and surface hardness, keep moving
  • Work out what changed. Almost every case follows a jump in distance, a change of surface, new shoes, or a return after a break
  • Increase again slowly when it settles — roughly a tenth more per week, not a doubling
  • Check the shoes. Worn-out running shoes and hard ground together are the commonest cause
  • Strengthen the calf, not just stretch it. Calf raises and the inward-turning exercise on the sheet load the muscles that attach along that border, which is what stops it returning
  • Swim or cycle while it settles, so fitness does not disappear

Avoid this

  • Running through it because it eases after the first kilometre. That is the pattern that turns this into a stress fracture
  • Running on concrete and on the same camber every day
  • Rest alone for weeks. Complete rest settles the pain and does nothing about the cause, so it returns on the first run back
  • Strong massage directly on the tender bone edge
  • Ignoring a point of pain small enough to cover with one fingertip. That is a different problem

The questions everybody asks

Is this a stress fracture?

Usually not, but the distinction matters.

Shin splints ache along a long stretch of the inner shin, several fingers' width, and ease with rest. A stress fracture hurts in one small spot, hurts when you hop, and often hurts at night. If your pain is the size of a fingertip, come in.

Why did it start now?

Something in the load changed.

New runners, a sudden increase in distance, a change to harder ground, worn shoes, or coming back after a break — one of those is nearly always in the story. The tissue was not weak; it was asked for more than it had been prepared for.

Can I keep running?

At a reduced level, usually yes.

Complete rest is rarely necessary and rarely helps. Cut distance until you can run without pain the next morning, hold there for a week or two, then build slowly. Pain that persists at that reduced level needs examining.

Do I need special insoles?

Sometimes, and not as step one.

Footwear and training load fix the majority. If you have a flat foot that rolls in significantly, arch support can help, but it is worth correcting the shoes and the mileage first and seeing what is left.

On cost: if we suggest a scan, an injection or an operation, ask us what it costs and what happens if you do nothing. We will answer both. You should never feel pushed into spending money.

Consultation is ₹500 for a first visit and ₹300 for a follow-up, the same at both clinics. For anything else, ask at the desk or on WhatsApp before it is done — we will tell you the exact cost.

Should you come and see us?

A page is a starting point, not a diagnosis. If the pain is not settling, come in and have it looked at properly — bring any X-ray, MRI or CT films you already have.
